Online Catalog Problem Report for Bibliographic, Holdings, and Item Records

This Problem Report Form should be used to report errors in the cataloging, holdings or item records for all types of materials processed by Cataloging & Serials Department staff. (If necessary, please print the form and send it with the item in question.) To report a journal issue that has not been received, please use the Claims Form. Report the following types of errors here:

• typos on bibliographic records
• typos in call numbers or holdings statements
• form of names or corporate bodies
• URLs that are incorrect
